Media Consent Forms
Throughout the school year, our students will have the opportunity to have their photo posted on our school portal, in local newspapers, and even on television.  As per NYC Department of Education Policy, a consent form must be signed by the parent/guardian and kept on file at the school.  If you have not done so yet, please download and complete the form.  Please return the form with your child to his/her teacher.  Thank you.
