School Food
The mission of the Office of SchoolFood is to provide safe, attractive, appetizing, nutritionally sound meals in a timely manner to the school children of the City of New York.
Lunch Forms

    All students were provided with a school lunch form, which must be completed and returned to school by October 2, 2009.  Please follow the following instructions when completing the form:

    * One school lunch form must be completed per household for all students attending the same school.
    * Please use black or dark blue ink to complete the form.
    * Do not cross out anything.
    * Do not use white-out.
    * Please provide all required information, and sign and date the form.

    You must obtain a confirmation number when you submit this application on-line.  Please provide this confirmation number to Ms. Lynn Gremmell in the main office.

    The information contained on the lunch forms will determine whether families will receive lunch for free, or at a reduced or full price.  All families must complete this form, regardless of whether or not your child will eat lunch provided by the school.  The information is also used to determine levels of government funding for your child's school.
