02G	Chicken nuggets registered at 48F in walk-in refrigerator. Chocolate milk registered at 41F degrees in milk chest.	Food items were immediately removed and discarded. Proper cooling procedures were implemented.
04N	One live roach observed on wall next to hand wash sink in food prep/service area.	OSFNS Staff re-instructed on proper cleaning and sanitizing procedures. All areas were immediately cleaned. Pest Control was immediately notified. RESOLVED.
