REQUESTS FOR VERIFICATION OF SERVICE
In order to facilitate the ability of the Division of Financial Operations to research and respond to requests for verification of service in a timely manner, please review the guidelines outlined below and follow the procedure indicated for the type of information you are requesting.
All requirements must be clearly outlined on the "Request for Verification of Service" Form and forwarded to the appropriate retirement system, office or current employer for authorization and submission to the Employment Records Research Office of the Division of Financial Operations. Responses will be provided directly to the authorizing agency or office. Requests for verification of service will not be accepted directly from employees.

Q: What if I need verification of service information while I was employed at the Department of Education but was not part of the pension plan?
A: Forward the completed “Request for Verification of Service” form to your current retirement system for authorization.
Q: What if I need pension information, i.e., membership dates, contributions, buy back information, tier status?
A: Contact your retirement system for this information.
Q: What if I need verification of service for an agency other than the New York City Department of Education?
A: Forward your request to the appropriate agency.
Q: What if I need verification of service rendered in a Day Care Center, Child Care Program, an “At Home” program or as a Community Liaison?
A: Contact the Department of Social Services unless the position was specifically funded by the Department of Education.
Q: What if I need verification of service during employment at CUNY?
A: Contact the Board of Higher Education.
Q: What if I need a copy of a teacher’s complete service history (Employee Profile)?
A: Contact Staffing Services at 65 Court Street, Room 816 for a copy of your Employee Profile.
Q: What if I need research or correction of information indicated on an Employee Profile?
A: Complete the “Request for Verification of Service” form and indicate the discrepancy in the comments box. Once your service has been verified, forward the form to Staffing Services, 65 Court Street, Room 816 to have your Employee Profile updated.
