IMPORTANT NOTE:

Please be advised that the Teacher's Choice Program, which is funded by the City Council, has been suspended for the upcoming school year 2011-2012 due to budgetary constraints. You may wish to refer to the UFT website for additional information.

All past participants who are direct payment (debit) card holders are reminded that they should activate their existing cards from SY 2010-2011 immediately if they have not yet done so, and to spend down any remaining funds on the card before the card expires. Please note that many cards are due to expire in December 2011. Also all cards received must be activated within a year of issuance in order to remain valid until expiration. Otherwise funding on the card will expire and can not be replenished.

If anyone has lost his/her debit card they should call the Chase Customer Service number at 866-795-3890 to request a replacement card. Once the replacement card is mailed to your home address, you should immediately activate the card by calling the Chase Customer Service number provided.

Please refer to the Teacher's Choice Direct Payment Card Holders document for more detailed information regarding the use and activation of your FY'11 debit card, if you have not already utilized your previous Teacher's Choice funding for last year.
