H1N1 Flu Vaccinations being given in school

Dear Families of P.S. 39,

The New York City Health Department is encouraging all parents to have their children vaccinated for H1N1 influenza.  This year the DOH is working with schools to offer the new vaccine at no cost to parents at elementary schools starting in late October/early November.

 Kindly read the entire booklet carefully.  If you wish your child vaccinated, please complete and return the application attached to the booklet to your child’s teacher in the envelope provided, no later than Thursday, October 22nd.  The application requires you to indicate your child’s OSIS number.  We will enter the OSIS number for your child.  Please make sure the front and back of the application is complete, signed and dated. The DOH will be requesting a tally of consent forms by Friday, October 23rd, so it is imperative that we receive your forms by Thursday.
